Telogen Effluvium: Shedding After Stress, Illness or Delivery

Telogen effluvium is a sudden, heavy, all-over shedding that begins about two to three months after a trigger, and recovers once the trigger passes and deficiencies are corrected.

Is this me?

  • Hair comes out in handfuls when washing or combing
  • It started roughly two to three months after an illness, fever, surgery, delivery or heavy stress
  • The shedding is all over, not in patches
  • Hair feels thinner overall but there are no bald spots
  • You lost weight quickly or changed your diet sharply
  • Full-length hairs with a small white bulb at the root

Why it happens

At any time most of your hair is growing and a small fraction is resting. A physical shock — a high fever, dengue or typhoid, COVID, surgery, childbirth, sudden weight loss, severe stress, a new medicine — pushes an unusually large number of follicles into the resting phase together. They stay there quietly for two to three months, then all shed at once. That delay is why people rarely connect the shedding to the event that caused it.

The important point is that the follicles are alive. Telogen effluvium is not baldness. Once the trigger has passed and iron, vitamin D, B12 and thyroid are corrected, the hair regrows. Treatment is therefore supportive: a mild serum, and all the suggested supplements, while the cycle resets.

When to consult first

Speak to the doctor first if any of these apply:
Sudden or patchy hair loss · scalp pain, sores, redness or oozing · scarring of the scalp · pregnant, trying to conceive or breastfeeding · under 18 · a heart or blood-pressure condition or medicines for it · hair loss getting rapidly worse.

What to expect, and when

Shedding typically continues for six to twelve weeks after it starts, then slows. Regrowth appears as short baby hairs along the parting at three to four months. Full density usually returns within six to twelve months. If shedding is still heavy after six months, chat with the doctor.

If your hair fall is not under control after a consistent three to six months, chat with the doctor rather than adding more products on your own.

Common questions

Will my hair grow back?

In telogen effluvium the follicles are alive, so yes — provided the trigger has passed and any deficiency is corrected.

Should I stop washing my hair?

No. Washing does not cause the shedding; the hair was already going to fall. Not washing makes the scalp worse.

Which tests should I get?

CBC, ferritin, vitamin D (25-OH), vitamin B12 and TSH. You can print the referral list from Build My Plan.

Do I need minoxidil?

Usually not for a pure telogen effluvium. Support and correction are the treatment.
